If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Failure to do so will result in fee of $30.00.
For patients treated for injuries from an automobile or work injury, we will attempt to receive authorization from the appropriate carriers before your visit. If prior authorization is not received, you will be responsible for the visit.
It is very important that your doctor have your current address, e-mail, work and home phone numbers, so we are able to contact you. Please provide us with an emergency contact number if possible. Please notify the receptionist of anychanges.
We would like to help our patients understand the difference between an office visit and a physical exam. This is especially important when we submit a bill to your insurance company for that visit. When scheduling an appointment, our office tries to figure out how much time you need with the doctor. Most of the time patients are seen as "office visits".This appointment time is used to discuss new or existing problems. The questions and exam will focus on the problems discussed.A "physical" or "preventative health exam" is a thorough review of your general wellbeing. The doctor will review your medical history, perform a complete physical exam and make recommendations regarding diet and exercise, age appropriate immunizations and screening exams for cancer like pap smears, prostate exam or breast exam and screening lab work.Ongoing chronic conditions and medications refills can be addressed as long as the condition is stable and does not require a significant change in treatment, your practitioner may choose to have you reschedule your physical and address the current conditions presented. Your Provider could choose to address both the problems and the physical at the same time. It is important to know that there would be two charges in this instance.It is important to know your benefit coverage prior to scheduling a physical or other preventative visit. Our office can not code for coverage and will only code according to services provided at the time of service.
Please contact your pharmacy and they will send us electronically or by fax your request for a refill. Allow up to 24 hours to process your request.
If the Doctor has to fill out forms regarding your care for administrative purposes, you may be charged $25. These forms should be returned to you within 10 business days.
Our goal is to provide you with your personal health record information which gives you 24/7 online access. Your personal health record contains your medical history, including diagnosis, medications, immunizations, appointments, allergies and laboratory test results. By providing us with your email address your doctor will grant you access to www.my.patientfusion.com, the web site used to accomplish this. You will receive through your email an invitation from www.my.patientfusion.com. An 8 letter access code will be given to you at our office that you can use to set your own login and password. Also you will get automated reminders of your appointments to the e-mail provided to us one week and one day before. Be aware that Nora Medical Group does not communicate by email with our patients to discuss labs or clinical questions.
